As a company, The Heart Team (a.k.a. The Heart & Vascular Team, PLLC) has ceased practicing medicine in Nashville, TN. With the exception of Dr. Brunsting, all of the other physicians are still practicing medicine in the Nashville area. Patients of Dr. Binford, Dr. Lancaster, or Dr. Walton, please contact their offices using the phone numbers provided above.
Patients of Dr. Brunsting with dates of surgery between January 1, 2001 and August 31, 2010, please contact Richards & Richards Office Records Management, Inc., 1741 Elm Hill Pike, Nashville, Tennessee, 37210, 615-242-9600 for assistance obtaining a copy of your records. Records for patients with dates of surgery prior to January 1, 2001 are not available and have been securely destroyed.
Patients of Dr. Swan should initially contact Dr. Swan's office to determine whether their records are with his current practice or stored with Richards & Richards. If they are stored with Richards & Richards Office Records Management, Inc., please contact them at 1741 Elm Hill Pike, Nashville, Tennessee, 37210, 615-242-9600 to obtain a copy of your records.
